Important information regarding the OHSAA physical forms

The OHSAA has partnered with a company that provides a data collection service in conjunction with the pre participation physical examination form (PPE). It is not necessary to go through the process that the OHSAA has posted on their website. All athletes must submit their signed physical forms prior to participation as they always have. The PrivIt Profile system will simply be an added step for you and at this time we do not believe that asking you to complete this extra step in the process is necessary or desirable. If you try to go through the Privit system you will not be able to complete the questionnaire as none of the Dublin schools have subscribed to the service.
