FinalForms isan online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your student(s). FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time!
Those of you new to The Dublin District will have to create an account and start from the beginning. Former Dublin middle school student athletes and Coffman returning student athletes should only have to review your information once per school year to verify information is current and sign the required forms.
*DON’T FORGET*Each student athlete will have a separate email address from their parent and will be required to sign some forms electronically.
All parents of athletes will be required to use FinalForms.
